1. Our Shared Goal

2. What the Adopter Agrees To

By signing below, I promise to:

Provide [Pet's Name] with love, care, food, water, and shelter.
Give appropriate veterinary care, including vaccines and check-ups.
Keep [Pet's Name] as a companion animal, not for resale, breeding, or experiments.
Contact the Rehomer if, for any reason, I can no longer keep [Pet's Name].

3. What the Rehomer Agrees To

By signing below, I promise that:

[Pet's Name] is being transferred voluntarily and to the best of my knowledge is healthy (unless noted below).
I have disclosed any known behavioral or medical issues.
